Important Notice About Requesting Form 1095-B

UHA does not automatically mail federal Form 1095-B to UHA subscribers. According to the IRS, Form 1095-B is not required for the annual filing of your federal tax returns. The IRS explains there is no longer a penalty (known as the individual shared responsibility payment) if you do not have a full-year of health care coverage. The information in Form 1095-B, therefore, is not necessary for current federal tax filings.
