  • build a strong relationship with your health care providers;
  • play a key role in staying healthy; and
  • have a way to address your problems.

Your Rights

Information Disclosure

You have the right to get correct and easy to understand information about:

  • your health plan
  • health care providers
  • health care facilities

You will be helped to make informed health care decisions, if you:

  • speak another language
  • you have a physical or mental disability
  • you don’t understand something

Choice of Providers and Plans

You have the right to a choice of health care providers and access to appropriate high-quality health care.

Access to Emergency Services

You have the right to visit an emergency room if you believe your health is in serious risk due to:

  • severe pain
  • an injury
  • sudden illness

You have the right to receive screening and stabilization emergency services:

  • when and where needed
  • without prior authorization

Respect and Nondiscrimination

You have the right to considerate, respectful, and nondiscriminatory care from your:

  • doctors
  • health plan representatives
  • other health care providers

    Confidentiality of Health Information

    You have the right to:

    • talk in confidence with health care providers
    • have your health care information protected
    • review and copy your own medical record from providers

    You may also ask your doctor to amend or append your record if it is not:

    • accurate
    • relevant
    • complete

      Complaints and Appeals

      You have the right to a fair, fast, and objective review by UHA of certain complaints you have against:

      • your health plan
      • doctors
      • hospitals
      • other health care staff

      This includes complaints about:

      • waiting times
      • operating hours
      • conduct of health care staff
      • adequacy of health care facilities
      • quality of your care or outcomes

      Your Responsibilities

      You owe it to yourself to have better health and a better life through your lifestyle choices such as:

      • exercising
      • not smoking
      • eating healthy foods

      Respond openly, honestly and completely to inquiries from your health care providers about your health.

      Get to know your health plan benefits and take advantage of the benefits that maintain wellness.

      Do your part to reduce health care costs by:

      • choosing a primary care physician to help you manage your health
      • completing an annual physical exam
      • reducing overuse of emergency room services by visiting your doctor or an urgent care clinic for conditions
