Your UHA plan allows you to meet with a Certified Diabetes Educator (CDE), Registered Dietitian or Certified Nutrition Specialist to learn more about Diabetes. - Generic co-payment on most diabetes medications1
Refer to the UHA Preferred Drug Listing to see if your medication is covered. - No co-payment on supplies
To Enroll/Participate
- Members with test results that indicate they have diabetes, are invited to participate. The program is open to all members.
- If you are interested in taking advantage of this program, or for any questions, contact Health Care Services or talk to your Personal Care Physician(PCP).
Eligibility and Fees
- All members are eligible
- No fees if seeing a participating "diabetes management program" provider
- There may be a co-payment on some medications¹
