These include, but are not limited to: Groups comprised of only one business owner/employee and his/her dependents will not qualify; UHA must be offered as the exclusive plan; Rates based on a 12-month contract; Open enrollment annually; Medical questionnaires required for all subscribers; No pre-existing conditions; Group must have between 2 to 50 employees; No coverage for out-of-state residents or employees; Subscribers 65 years or older must have Medicare Part A; Groups with less than 10 employees must automatically deduct premiums.
