- Annual open enrollment
- Loss of other medical coverage (within 31 days of loss of coverage) *
- Newborn (within 31 days of birth) *
- Adoption (within 31 days of adoptive placement) *
- Marriage (within 31 days of marriage) *
Requests for enrollment at any other time cannot be accommodated. No retroactive enrollment is accepted.
